Our Team
Some BlueCross BlueShield policies, Medicaid, and TRICARE Prime require a referral from your primary care physician. Patients on UnitedHealthcare Medicare Advantage plans and Cigna HealthSpring plans can only see Dr. McLeod.Some BlueCross BlueShield and Viva plans are restricted to specific groups of providers, which we may not be a part of. Your insurance company can provide you a list of participating doctors.
Payment is expected at the time services are rendered. Co-pays, deductibles, and balances are due at check-in. As a courtesy we will file your insurance claim, but the responsibility of insurance verification for office visits, procedures, and surgeries lies with the patient. As a courtesy we will make every effort to provide you with a good faith estimate of your portion.We accept MasterCard, Visa, Discover, and American Express. Financing is available with approved credit through Care Credit (GE Moneybank).

Please be sure to bring all your insurance cards, any copays due, and an updated list of medications. If you are coming for CPAP check-up, bring your data card from your machine.For established patients, we offer same-day appointments for urgent sinus and allergy problems if you call before 9am.
As of March 1, 2020, there will be a charge of $30.00 to fill out FMLA and Disability forms. The $30.00 will be due when the forms are dropped off. We will complete the forms within 10 business days.
